Dr. José María Vilar Sánchez

Dr. Vilar is a specialist in obstetrics and gynaecology and currently carries out his expert medical work at the University Hospital of Jerez de la Frontera. From 2013 to 2019 he belonged to the leading group in assisted human reproduction, the Valencian Institute of Infertility (IVI), and he continues to work closely with this institution.

Functional Urology is the subspecialty that deals with the diagnosis and treatment of voiding dysfunctions in both women and men, urinary incontinence, pelvic organ prolapse, recurrent urinary tract infections and chronic pelvic pain, among other pathologies.

Recurrent urinary tract infections
Recurrent urinary tract infections are a very common problem in women. There are various treatments that can help prevent their appearance, such as prophylaxis with autogenous vaccines, use of probiotics, topical treatment with oestrogens, or even intravesical therapy with hyaluronic acid.
Urinary incontinence or male and female urination symptoms
Bladder dysfunctions such as overactive bladder or neurogenic bladder, as well as urinary incontinence, whether stress or urge, require individualised assessment and treatment. From pelvic floor rehabilitation therapy with techniques such as biofeedback, minimally invasive surgery for urinary incontinence such as the suburethral mini-sling or intravesical injection of botulinum toxin.
Chronic pelvic pain
An adequate diagnostic study of this pathology is essential to be able to establish the cause that produces it and thus propose the best treatment strategy for each patient.
Early diagnosis of prostate cancer and other urological tumours
Urological tumours can be numerous depending on the organ they affect (kidney, prostate, bladder, testicle...). The most common is prostate cancer, for which diagnostic tests are used to detect it such as multiparametric prostate imaging and fusion biopsy. The treatment of different urological tumours involves different types of surgery. Endoscopic treatment and laparoscopic surgery allow us to offer a less invasive treatment with a better recovery after the intervention.

Urodynamic Studies / Flowmetry / Manometry
Many voiding or anal dysfunctions are not anatomic problems but tend to fall into a definition of functional problems. We carry out functional diagnostic tests so that later the treatments can be adjusted to each case.
Pelvic floor rehabilitation
Incontinence can be improved or prevented if we strengthen the pelvic floor muscles. The biofeedback technique with electrodes can help us identify the key pelvic floor muscles for enhanced continence.
Cystoscopies and bladder instillations
This refers to a test that allows us "to see the bladder inside". Essential test for the diagnosis and monitoring of many bladder diseases. Some bladder diseases require treatment in the form of "washes" with chemical substances or drug solutions that have proven clinical efficacy.

The treatment of pelvic pain is carried out through the application of specialised techniques for pain control that are the speciality of our Pain Unit team.

Origin of pain
The use of analgesic techniques such as nerve blockers (infiltrations), are not only part of the treatment, but also help diagnosis by differentiating structures involved in pain.
Technique consisting of the administration of an electric current through an electrode that is placed inside a tissue to produce a biological effect (analgesia). It is a minimally invasive technique and to most patients on an outpatient basis.
This involves the selective stimulation of sacral roots for the treatment of perineal origin pain and pelvic organic dysfunction.
TENS and Electroacupuncture
Technique based on stimulating and neuromodulating the posterior tibial nerve through small electrodes on the skin, which in a retrograde manner will also stimulate all the pelvic structures described above.
